Description:  While BNSF Yard Job Y-FTW2021-16 was making a shove uphill (1% Grade) into BNSF North Yard track 9119, the trailing truck on the 5th car from the head-end (BNSF 563014) lifted up over the frog and derailed. Power was BNSF 3189, BNSF 3133, and BNSF 3163 shoving 3 loads and 36 empties. The switch was not designed to handle this type of equipment under this particular loading scenario. This photo shows the marks on the rail which indicate the point of derailment.
